Original documents (statements and logs) must be remitted (no emails, photocopies or faxes). Would you like an original form mailed to you, or would you like a form emailed to you for printing?

Please be aware that each complaint must be verified. Most complaints require completion of a statement or a log. Should you request a notice of violation (ticket) be issued based upon your complaint, please note that your written statements and documentation are used as evidence and may be presented in court. In order to issue a ticket, and if necessary to proceed to Court, they must be detailed and accurate. In order for the court to allow you to refer back to the forms when giving evidence they must be in your own handwriting and must be made in a timely manner. Each witness must complete their own forms and should contact us directly to request a form. Finally, any additional notes made on scrap paper should be retained for use in Court.
